What services does AJFC provide?
AJFC offers a variety of programs to support individuals and families, including utility assistance (LIHEAP), emergency food and shelter (EFSP), housing support, educational workshops, case management, transportation assistance, youth development programs (CSBG), and more.
Who is eligible for services at AJFC?
Eligibility depends on household income, family size, and specific program requirements. Most programs serve low-to-moderate-income individuals and families residing in our service areas. Proof of income and residency is required.

How do I apply for assistance?
Clients should use the automatic scheduler to book an appointment.
What documents are required when applying?
Required documentation may vary by program but typically includes:
Photo ID
Social Security cards for all household members
Proof of income (pay stubs, award letters, self-employment proof)
Current utility bills, lease agreements, or vendor invoices
Notarized zero-income affidavits or contribution statements, if applicable
What is the Contribution Statement Form?
If a client reports no income, they will be asked to complete a Contribution Statement Form to document how their household needs are being met. This form identifies anyone who may be providing financial help directly to the client. This form must be signed by the contributor in the presence of a notary.
What is the Zero Income Affidavit?
Clients with no income must submit a Zero Income Affidavit. The client completes the top section and must sign the document in front of a notary public. This form verifies that the client has had no income for a specific time period.
What is case management and how does it help?
Case management is a supportive service that connects clients with a designated caseworker who helps develop a personalized plan to reach self-sufficiency. This includes regular 30-day follow-ups, referrals to programs, assistance with goal setting, and monitoring progress. Services may include employment support, housing stabilization, education planning, and other wraparound services.

Why does AJFC require a lease agreement?
AJFC requires a valid lease agreement to verify your housing status and determine eligibility for housing-related assistance programs. This helps ensure funding is properly allocated and supports those in need of housing stability.
What should be included in a lease agreement?
A valid lease agreement must include:
Names of the tenant(s) and landlord
Property address
Lease term (start and end date)
Monthly rent amount
Signatures of both landlord and tenant
Utility information (whether included or not)
AJFC provides a general lease template if you or your landlord need one.
Does the lease need to specify if utilities are included?
Yes. The lease agreement must clearly state whether utilities (electricity, water, gas, etc.) are included in the rent. AJFC may request utility bills to confirm housing costs.
What is a landlord invoice and why is it required?
A landlord invoice documents the amount owed by the tenant and is required for payment processing. It should include:
Tenant’s name and property address
Monthly rent amount
Any utility or other charges
Total due
Landlord’s contact and signature
Invoice cannot be older than 30 days.
Invoices help AJFC verify costs and issue payments directly to the landlord if approved.
Can AJFC help me fill out the lease or invoice form?
No, The lease agreement should be completed by the Landlord.
What if I have a utility emergency or disconnect notice?
If you have received a disconnect notice or are facing a utility emergency, contact AJFC immediately. Our Crisis Intervention Program may be able to assist, depending on available funding and eligibility.
You must provide proof of the emergency, including:
A copy of the utility disconnect notice (must include name, address, and account number)
A valid lease agreement
Proof of income or a zero income affidavit
A completed AJFC application
Explanation of what caused the emergency (e.g., job loss, illness, family crisis, etc.)
Please note: Assistance is not guaranteed, and documentation must be current and accurate.
What if I have a housing emergency (eviction, unsafe conditions, etc.)?
If you are facing eviction, foreclosure, or unsafe living conditions, AJFC may be able to help with short-term support and referrals.
You must provide proof of the emergency, such as:
A formal eviction notice or court summons
A notice of foreclosure
Photos, inspection reports, or written documentation of unsafe conditions
A valid lease agreement
Explanation of what caused the emergency (e.g., financial hardship, loss of income, medical expenses, etc.)
Contact your AJFC caseworker immediately to begin the process. Additional documentation and eligibility screening may be required.
Does AJFC Community Action Agency accept donations?
Yes! AJFC gratefully accepts a wide range of donations including cash, clothing, vehicles, property (such as houses), and other items. Every donation directly contributes to our efforts in supporting individuals and families in the community. Your generosity helps us expand our programs, provide emergency resources, and offer long-term support to those in need.
